Steps to Adulthood service (Transitions)

Welcome to the new Steps to Adulthood Service (Transitions) in Haringey 

Haringey Children’s and Adults Services are excited to announce the creation of a new integrated transition service, the Steps to Adulthood Service, that will support young people transition to adulthood. The team will work with children with care and support needs, including those with SEND. The assessment will be completed with the young person and their parent carer.  The young person will then be supported within the Steps to Adulthood Team until they are settled into Adults support.

Who is it for?

Young people aged 14 to 25 who are most likely to need some ongoing support in adulthood. The new team has started by working with 17-year-olds and aims to begin working with 16-year-olds.

Who are the team?

Members of the assessment team come from a social work background. They’ll work with families and/or carers and with practitioners from a wide range of other disciplines.

What will they do?

The team members will work with young people, their families and schools by:

  • Carrying out Care Act Assessments to see if the young person will need social care support in adulthood
  • Carrying out Carer’s Assessments to support parents and carers to manage and sustain their caring responsibilities
  • Looking at options, for example about where the young person might live as an adult
  • Signposting young people to relevant services and support in the community, such as finding an educational course, work experience, or finding employment

How will young people access the service?

Young people are allocated to the Steps to Adulthood Team via the monthly Transitions Panel. This is a monthly panel where services within the Council present the cases of young people who will likely require care and support in Adults Services. If they need this support, they will be directed to the Steps to Adulthood Team. As soon as possible, a social worker will be allocated to the young person to start the assessment.
