Portage Home Visiting Service

Working with Children

  • Our Portage Visitors maintain a caseload of children, visiting them weekly or fortnightly.
  • They collaborate closely with parents and professionals to identify the child's developmental stage and understand family aspirations.
  • Modelling play ideas, lending toys, and suggesting activities help facilitate the child's progress while incorporating specialist advice into daily play.
  • We partner with the Specialist Health Visiting and Vision Support teams to host the Huddle, a supportive playgroup for children under 5 with SEND.
  • At the Huddle, families can access guidance on toileting, sleep, behaviour, learning, play, resources, and transitioning to the nursery, all within a fun and supportive environment.

Supporting Parents

  • Portage Visitors work alongside parents to help them navigate the early years and SEND systems.
  • They assist parents in recording the child's progress and effective strategies, ensuring smooth transitions to educational settings.
  • They support parents in engaging with community activities and resources and accessing available funding to promote comprehensive community involvement.

Collaborating with Schools and Settings

  • Our team assists parents in finding a suitable nursery or school for their child.
  • They work closely with the school, family, and other professionals to ensure the right support is in place.
  • Their goal is to help the child settle quickly and make progress in their new educational setting.
