What to do when you leave hospital

Please try to arrange for a family member or friend to collect you if possible. Transport is available for those patients the hospital team think need it because of medical need.

Ask someone to bring outdoor clothing on the day before you leave if you do not already have suitable clothing with you.

Make sure you

  • take all personal belongings with you when leaving the hospital
  • give back any hospital equipment you have, e.g. crutches
  • have the medication you need with you
  • collect your copy of your hospital discharge letter – this helps ensure your GP can continue to support you once you are home

If you do need onward care and support to help you recover, or you were receiving home care prior to admission, these arrangements will be discussed with you.
