We support children and young people 0 to 25 with special educational needs and disabilities. We aim to ensure they have the best possible start in life and opportunities to thrive. We’re committed to working in partnership with families, schools, and other agencies to provide a coordinated approach to meeting the needs of children and young people with SEND.
Our key responsibilities:
- assessment and identification: We work with families and professionals to identify children and young people with SEND, aiming to provide timely and accurate assessments of their needs
- Education, Health and Care Plans (EHCPs): We coordinate the development and review of EHCPs, which outline the support required to meet the educational, health, and social care needs of children and young people with SEND
- support and intervention for schools: Our team offers guidance and assistance to schools and settings, helping them to provide appropriate provisions for children and young people with SEND
- multi-agency working: We collaborate with health, social care, and other services to ensure a holistic approach to supporting children and young people with SEND
- transition planning: We support effective transition planning at key stages of education, particularly the transition into school and from school to adulthood
- information and guidance: We offer information and guidance to families and professionals about SEND processes and provisions
