Your Support Plan
Your support plan should put you in control of how your care and support needs will be met. This should enhance your independence and wellbeing.
We will work with you, your family or your carer to identify what is important in your life. We will set out your choice/s of support to help you meet the goals identified in your assessment.
Regular support plan reviews
We will stay in touch with you to ensure that your needs are being met and that the services you receive are keeping you safe and well.
Once we are sure that things are going well, we will have a review of the support plan at least once a year. We will make sure that you are managing your direct payment, and to see if your needs have changed
