Prevention and early intervention
It is more effective and cost-efficient to intervene early and prevent issues from escalating. We will identify and address risk factors, and build protection. We will empower individuals to lead healthy and fulfilling lives through:
- early identification and assessment: collaborating with partners across health, education, housing, and community sectors to identify signs of vulnerability and intervene early
- targeted interventions: psychoeducation, skill-building programmes, family support services, and community-based initiatives to address underlying risks and strengthen protective factors
- partnership working: work with health agencies, children services, community organisations, and voluntary groups. Coordinate and deliver integrated prevention and early intervention services. By pooling resources, expertise, and networks, our interventions will be better and reach those most in need
- promotion of resilience and empowerment: building on individuals' strengths, assets, and resources to enhance their capacity to cope with challenges and changes
- community engagement and participation: involving communities in the design, delivery, and evaluation of services. This will build trust, nd ensure that services are culturally relevant and meaningful
- data-driven decision-making: collecting and analysing data on risk factors, service utilisation patterns, and outcomes to continuously improve and refine our strategies
- promotion of a system-wide approach: by integrating prevention and early intervention into policy, practice, and commissioning processes, we aim to create a more responsive, coordinated, and sustainable social care system
Prevention and early intervention will reduce the need for higher-cost interventions, and enhance the wellbeing of individuals and families within our community.
