We offer comprehensive assessments, personalised care planning, and access to a range of community-based support services. we aim to empower you to live safely and comfortably in their your own home, surrounded by familiar surroundings and social connections.

Central to our Home First model is the integration of health and social care services. Primary care, community services, and social care providers should work together. We want to break down silos and adopt a multidisciplinary approach. We want you receive holistic, coordinated support that addresses your physical, emotional, and social needs in a person-centred manner.

We want to make more use of innovative technologies and telecare solutions. These can enable remote monitoring, communication, and support. This can enhance safety and security for people living at home. It means you can get help when it's needed.
