You can ask a carer or trusted friend to help you.

It is very important that the Hospital passport has got the correct information in it about your health and is up to date.

If you need to stay in a hospital, show the doctors, nurses, and x-ray staff your Hospital passport (if you do not have a purple folder).

The hospital passport will help you tell them about how to support you if you need to go into hospital.
